Pet Carer Engagement Letter

Date: 13 December 2025

1. (MMC Carer ID: ) (the “Pet Carer”) acknowledges and agrees that this Letter incorporates by reference and is subject to:
   1. the Pet Service Agreement dated (Reference: PETSERVICE-);
   2. the Pet Carer Agreement dated (Reference: CARER-); and
   3. the Service Order dated (Reference: ORDER-).
      
2. The Pet Carer:
   1. expressly agrees to be bound by all terms, conditions and obligations set forth in the referenced Pet Service Agreement; and
   2. acknowledges that execution of this Letter constitutes their formal accession as a party to the Pet Service Agreement in respect of this specific engagement.

IN WITNESS whereof, by accepting the work, the Letter is executed and delivered on the date of acceptance.
