Mind My Cat is a professional pet care platform committed to serving pet owners and their furry companions, in which safeguarding security and trust is of paramount importance to us. To uphold the integrity of our community, we require verification of all members, including both Pet Owners and Pet Carers.

All personal data collected will be handled in strict compliance with the Personal Data (Privacy) Ordinance (Cap. 486 of the Laws of Hong Kong). The manner in which such data is managed is set out in our Personal Information Collection Statement (link below).

I certify that the information provided on the Application is true and accurate. I understand that any misrepresentation or omission of material facts may result in the Application being declined and immediate termination of the engagement.

In exchange for the consideration of the Application by the Company, I hereby agree as follows:

1. I acknowledge and agree that, if selected, my engagement with the Company shall be solely on an independent contractor basis, and not as an employee. Neither the acceptance of the Application nor the subsequent engagement shall be deemed to create an express or implied employment contract or relationship subject to the provisions of the Employment Ordinance (Cap. 57 of the Laws of Hong Kong) (the “EO”). I further understand that as an independent contractor, I am not entitled to employee benefits, protection or rights under the EO or any other employment-related statutory scheme, unless expressly provided for in a subsequent, mutually executed agreement(s).

2. I authorise the Company to verify the information provided in the Application, among others, contacting institutions and other relevant parties. I understand that such verification shall be conducted in compliance with the Personal Data (Privacy) Ordinance (Cap. 486 of the Laws of Hong Kong) (the “PDPO”) and any applicable laws. I consent to the collection, processing, and retention of such information strictly for the purpose of assessing my suitability for the position. I further acknowledge that this data will be handled in compliance with the PDPO and other applicable legal requirements, and will be maintained in a secure and confidential manner, disclosed only as mandated by law.  I further understand that the Company may conduct background checks, including inquiries into personal reputation and lifestyle.                            

3. Recognising the responsibilities inherent in the role of pet care, including entering clients’ residences and caring for their animals:
   
   I understand that I am required to disclose any criminal convictions that are relevant to the position.  For positions involving the handling of animals with medical needs or the performance of strenuous physical activity, I acknowledge that the Company may request verification of my ability to perform the required duties. 

4. I acknowledge that submission of the Application does not guarantee acceptance and the Company may require additional information at its discretion.
